Protected Health Information
As a dental practice, we are a covered entity under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). Our use and disclosure of PHI is governed by our Notice of Privacy Practices, which is provided to every patient and is available at our office upon request. To the extent this Privacy Policy conflicts with our Notice of Privacy Practices regarding PHI, the Notice of Privacy Practices controls. Text Messages and Phone Calls
If you provide your mobile phone number and agree to receive text messages from us, we may send you appointment confirmations, reminders, follow-up messages and, where you have opted in, promotional messages about our services. Message frequency varies. Message and data rates may apply. You can opt out of text messages at any time by replying STOP to any message we send, or by contacting our office. Reply HELP for assistance. Consent to receive text messages is not a condition of receiving dental care.
Your mobile phone number and your consent to receive text messages will not be shared with or sold to third parties for their own marketing purposes. Phone numbers are shared only with the service providers we use to send messages on our behalf.
How We Share Your Information
We do not sell your personal information. We may share your information in the following limited circumstances:
• Service providers. With vendors who perform services on our behalf, such as website hosting, online scheduling, patient communication and reminders, email and text messaging, analytics, and advertising. These providers are permitted to use your information only to perform services for us.

We maintain a written information security program consistent with the Massachusetts Standards for the Protection of Personal Information (201 CMR 17.00) and the HIPAA Security Rule. This includes administrative, technical and physical safeguards designed to protect personal information against unauthorized access, use or disclosure, such as encryption of data in transit, access controls, staff training and oversight of our service providers. No method of transmission over the internet or method of electronic storage is completely secure, so we cannot guarantee absolute security.

Your Choices and Rights
Marketing communications. You may opt out of promotional emails by using the unsubscribe link in any marketing email, and out of text messages by replying STOP. We will continue to send you transactional messages, such as appointment confirmations, unless you tell us otherwise.
Access and correction. You may request to review, update or correct the personal information we hold about you by contacting us using the details below. Requests relating to your dental records and other PHI are handled under our Notice of Privacy Practices and HIPAA, which give you rights to access, amend and receive an accounting of certain disclosures of your PHI.
Massachusetts residents. Massachusetts law requires us to protect your personal information and to notify you of certain security breaches. If you have questions about how we protect your information, please contact us.
Children’s Privacy
The Site is intended for use by adults. We do not knowingly collect personal information online from children under 13 without verifiable parental consent. Parents and legal guardians may provide information about a minor child for the purpose of scheduling and receiving dental care. If you believe a child under 13 has provided personal information to us through the Site without consent, please contact us and we will delete it.
Third-Party Websites
The Site may contain links to third-party websites, including online scheduling, patient portal, insurance and social media sites. We are not responsible for the privacy practices or content of those sites. We encourage you to review the privacy policy of any website you visit.
